HR News You Can Use

Health Risk Assessment 2023

It’s Health Risk Assessment time at NCMIC! This year, HRAs will not be held onsite. Employees are asked to see their primary care physician to complete the HRA. If you cannot see your primary care physician, employees are able to schedule an appointment with Medicap to complete their HRA.

As a reminder for 2024, all spouses who have health coverage elsewhere, will need to come off NCMIC’s plan. If your spouse will be staying on our plan, they will need to complete a HRA, as well. If you have any questions regarding this change, please contact HR.

Employees should have received an email to their personal email account with the online portion of the Health Risk Assessment. Spouses do not need to complete this portion, even if they will be staying on our health plan.

Those employees and covered spouses/significant other who do not complete the Health Risk Assessment and/or are a tobacco user will be subject to a premium surcharge in 2024. The 2023 Tobacco Affidavit form will need to be filled out and turned into HR. Any dependent 18 or older that is covered on the medical plan will also need to complete the form.

Check your inbox for more details. All forms were sent to employees on Aug. 28, 2023. Health Risk Assessments and online forms need to be completed and sent to MercyOne (not HR) by Nov. 1, 2023.

MercyOne Flu Shot Clinic

MercyOne will be onsite hosting a flu shot clinic for employees and spouses on Tuesday, Sept. 26, 2023 from 9:30–11:30 a.m. in conference rooms 122/123.

Employees received an email on Sept. 12 that included the flu shot consent form that needs to be completed prior to your appointment. Sign up is also necessary. Please use this link to reserve your time and bring the completed consent form with you to your appointment.
